Beat someone ranked above you and you climb - there's no reward for farming easier opponents, so the ladder stays fair for newer players too. Every submitted round also sharpens the course difficulty ratings - the more people play, the more accurate they get, for everyone.
The first time you join a queue, you'll pick a name and a short PIN. It's not a real account - just enough to stop two people accidentally using the same name, and to stop someone else casually playing under your name from a different device. You'll only be asked once per browser - after that, this device remembers you and won't ask again. If you ever play from a different phone, computer, or browser, use the exact same name and PIN there too, so it's still recognised as you and your ladder position carries across.
